At Crosshope, your therapist and case manager work with you to develop an aftercare plan based on your recovery goals, living situation, medical needs, and available support. We help arrange follow-up appointments, continued medication care, and connections to community resources before you leave treatment.
Aftercare coordination is the process of organizing the treatment, appointments, and practical support you may need after completing a structured addiction treatment program. Instead of leaving treatment with a list of recommendations, you work with your treatment team to identify the next steps and connect with the people and services that can help.
At Crosshope, you receive aftercare coordination when you complete our Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), or Outpatient Program (OP). It isn’t a separate service you need to enroll in after treatment.
Your aftercare plan may address several areas of your life, depending on what you need:
Each part of your plan includes a specific resource and a next step, so you know who to contact and what needs to happen after treatment.
Your aftercare plan may include continued treatment at Crosshope and referrals to outside services in Pine Bluff or other Arkansas communities. We help arrange the support you need before you leave treatment, so you know which appointments, resources, and next steps are already in place.
Some clients continue individual therapy through our outpatient program at a lower frequency after completing more intensive treatment. Family sessions may also continue when appropriate and with your written consent.
If you receive Medication-Assisted Treatment (MAT) or psychiatric medication management, you may continue seeing the same prescribing providers after structured therapy ends. Your team helps arrange these appointments so medication care doesn't stop when your treatment schedule changes.
Recovery meetings can give you people to talk to outside your treatment appointments. Depending on your preferences, we can help you connect with Alcoholics Anonymous (AA), Narcotics Anonymous (NA), SMART Recovery, faith-based groups, or peer recovery support.
Your case manager can help identify meetings that fit your location, transportation options, and schedule. These groups come from outside organizations, but we can help you make the initial connection.
Stable housing, reliable transportation, and work or school responsibilities can affect your ability to follow an aftercare plan. If you need help in these areas, we can coordinate referrals to recovery housing, sober living homes, Arkansas Workforce Centers, or adult education programs.
We can also assist with Medicaid renewals, SNAP applications, transportation planning, and other practical needs. When an outside organization provides the service, we help with the referral and follow up rather than leaving you to manage the process alone.
Your aftercare needs may extend beyond addiction treatment. We can help coordinate primary care, dental services, specialty medical appointments, and prescription assistance when needed.
If you have legal or court-related obligations, we can also assist with treatment documentation and coordination involving probation, parole, drug court, or other agencies. Information is shared with outside providers or organizations only with the required authorization.
Aftercare coordination is included in our PHP, IOP, and OP treatment programs, so you don’t need to arrange separate coverage just for aftercare planning. Our admissions team can verify your insurance benefits, explain what your plan covers, and help you understand any expected out-of-pocket costs.
Some services you continue after treatment, such as outpatient therapy, medication management, or appointments with outside providers, may have separate coverage requirements. We’ll help you identify those costs and coverage questions as part of planning your next steps.

Tell us what you need help with and what is happening now.
Our team determines your level of care and starts planning for recovery.
Begin treatment with an aftercare plan that evolves as you progress.
Leaving structured treatment doesn’t mean you have to manage everything on your own. We stay connected after discharge, check whether your plan is working, and help you adjust when life or your recovery needs change.
Your primary therapist reviews your progress, recovery goals, and the challenges you may face after treatment. We look at your current substance use risks, mental health needs, living situation, and ability to attend ongoing appointments.
The team also considers what worked during treatment and where you may still need support. Each part of your aftercare plan identifies a resource and a next step with a date, so you leave knowing what needs to happen and when.
Our case manager or therapist typically contacts you during the first week after discharge, then again around 30, 60, and 90 days. These check-ins give you a chance to discuss how things are going, whether you're attending appointments, and whether any new problems have come up.
If you're having trouble following your plan, we can review what is getting in the way and help arrange additional support. Follow-up may continue beyond 90 days when your needs call for it.
We don't write an aftercare plan once and leave it unchanged. Missed appointments, medication problems, housing changes, or new stress at home or work may mean you need a different kind of support.
Your therapist or case manager reviews these concerns with you and helps determine the next step. That might mean adding therapy appointments, addressing a practical barrier, or arranging a new clinical assessment if your recovery becomes less stable.
If you return to substance use or begin struggling after discharge, you can contact us for help. Our clinical team can typically complete a reassessment the same day or next day to determine whether OP, IOP, or PHP is appropriate.
You don't have to restart the entire admissions process. If you need medically supervised withdrawal management or residential treatment, we can help coordinate placement with an appropriate outside facility and plan for your return when clinically appropriate.
Schedule a confidential consultation to discuss your needs and explore the right treatment options for you.
Yes, but you'll need to complete an assessment and enter an appropriate treatment program at Crosshope. If you've finished detox, residential, or inpatient treatment elsewhere, our clinical team can determine whether our IOP or OP is appropriate for your next stage of care. Those programs include aftercare planning. We don't offer aftercare-only coordination as a separate service.
Yes, with your written consent. Family members can participate in discharge planning, learn about warning signs, and discuss how to support you at home. We can also arrange family sessions when clinically appropriate and connect loved ones with resources such as Al-Anon or Nar-Anon. You decide what treatment information we can share, subject to applicable privacy requirements.
Our scheduled follow-up typically continues for about 90 days after discharge, with additional contact when your needs call for it. Medication-assisted treatment and psychiatric medication management may continue longer. There's also no deadline for reaching back out. Former clients can contact our team whenever they need help reconnecting with treatment.
Yes. Your aftercare plan considers your work or school schedule, transportation, and ongoing treatment needs. Before discharge, we review which appointments and recovery activities you need to maintain and help you plan around those responsibilities. If your schedule makes it difficult to attend recommended care, we'll discuss available options rather than simply removing necessary support.
Tell your therapist or case manager as early as possible. We can help you explore sober living and recovery housing options in Pine Bluff and other Arkansas communities, assist with applications, and coordinate referrals based on your needs. We consider housing availability, eligibility, and your ability to travel when planning your next steps. We cannot guarantee placement, but we can help you understand your options before discharge.
Contact your case manager so we can review how losing coverage may affect your appointments, medications, and ongoing treatment. We can help with Arkansas Medicaid applications or renewals, explore prescription assistance programs, and coordinate with providers about available options. Your eligibility and coverage will determine which services you can continue using.
Pine Bluff has several Alcoholics Anonymous (AA) meetings. You can find current locations and schedules in the Arkansas Central Office AA meeting directory.
If you prefer Narcotics Anonymous (NA), SMART Recovery, or another type of peer support, our case manager can help you explore local and online options. We'll consider your preferences, transportation, and schedule when helping you find meetings you can continue attending after treatment.
Leaving treatment can bring a mix of excitement and uncertainty. You may feel ready to move forward but still wonder how you’ll handle difficult days, keep up with appointments, or find support when you need it. Those are things we can help you prepare for.
At Crosshope, we start planning for life after treatment from the beginning, so you won’t have to make those decisions alone when it’s time to leave. Whether you need continued care, help finding community resources, or someone to turn to when things get difficult, we’ll work with you to put a plan in place.
If you’re thinking about addiction treatment in Pine Bluff, Arkansas, please call us. We’ll listen to what you’re going through, answer your questions, and help you take the next step with a clearer idea of what comes after treatment.
