Website forms. When you submit a callback or contact form, we collect your name, phone number, email address, whether you have insurance, and anything you write in the message field. Please do not include detailed medical or treatment history in a website form. Those details belong in a phone call or an intake conversation.
Phone calls. We use a call tracking service to measure which pages and campaigns lead to calls. When you call a number on this site, the service logs the number you called from, the time, and the length of the call. Calls may be monitored for quality or training.
